Insurance and Payment Information
Insurance information displayed on the website is not a guarantee of coverage or payment. Verification of benefits is not the same as authorization for treatment and does not guarantee that an insurer will pay a claim. Coverage, deductibles, copayments, coinsurance, exclusions, medical-necessity determinations, and other obligations are controlled by the applicable health plan and the person's financial agreements.
You are responsible for confirming benefits and understanding any financial responsibility associated with services.
Communications
